Secondary schools within the Scenic Rim Region participate in the annual Queensland Health School Immunisation Program provided by the Scenic Rim Regional Council. In 2017, all year 7 students will be offered the following vaccinations (free) as recommended by the National Immunisation Schedule:
· Human Papillomavirus (HPV); and
· Diphtheria,Tetanus,Pertussis (whooping cough)
A team of registered nurses will visit participating schools periodically throughout the year to conduct the immunisation clinics.
Upon commencement of the school year, each school will distribute a consent form pack to all eligible students to take home.
If you would like your child to be vaccinated, it is very important to read the information carefully, complete, sign and return consent form/s to your school's administration office as soon as possible.
Please take special care completing the consent form:
· Ensure all sections are completed
· Ensure all text (including school name) to be written 'in full';
· Ensure correct date and year of child's birth; and
· Ensure Medicare number is completed correctly (see below for example)
Only students who have returned a completed and signed consent form will be vaccinated.
If you do not wish your child to be vaccinated, or your child has already received vaccination, please return the consent form completing the NO section.

Download and print required form (in colour), read the information carefully, complete, sign and return consent form to your school's administration office as soon as possible.
It is important to have your child optimally protected against a number of vaccine preventable diseases. If your child does miss any immunisation offered during the school year due to absenteeism, illness or refusal they can still be immunised by way of catch up.

Parents/Legal Guardians requesting evidence that their child was vaccinated as part of the School Immunisation Program (SIP) can request this information from the service provider. Such requests should be made in writing in order to adhere to privacy legislation.
Scenic Rim Regional Council is the service provider for the SIP within the Scenic Rim Region. Should you require a copy of immunisation records, you can request immunisation records for yourself or your child.
To request an immunisation record, please complete and submit the following immunisation records request form via one of the following methods.
Please note there is no fee associated with this request.
The request should include:
- Your full name;
- Your current address; and
- Your contact phone number/s.
In addition to the above, if the request is not for yourself, what is:
- Your relationship to the person whose records you are requesting; and
What are the following details of the person detailed within the record:
- Full name;
- Date of birth;
- Current address;
- Address (at time of vaccination);
- School attended (at time of vaccination); and
- Year/Grade (at time of vaccination).
